1. For new membership application, payment must be made PRIOR TO FORM SUBMISSION.
2. Proof of payment must be attached together with Registration Form.
3. The ONLY accepted payment method at this moment is via online bank transfer.
4. Payment has to be made SEPARATELY for each membership application.
5. MySLPM reserve the right to reject application that to do conform to the terms and condition stipulated.
